Is it tight fitting ?

It is very comfortable. Not tight if the size is right.

How do I make sense of the sizing when it is done in two sets of numbers for each size?

First number is for waist size, second is for hips size

How is this item secured to your body? Velcro? Is it sturdy? Will be using during high movement activity...

I love this back warmer. It is very comfy right up against your skin- not itchy at all. It is sewn together. Flexible and tight to your body so you can pull it on over your head. I use it for contortion training as well as just to stay warm under my regular clothes. It will definitely stay on during your activity sometimes it slides a little.
